    Job Description:

    The Nurse Practitioner- Women's Health supports the Women's Health, Prenatal, and Postpartum Services of CHC. This position is responsible for the initial prenatal visit and overall women's health care services; including pre-conception care, inter-conception care, post-partum care, gynecology, and contraceptive management.

    This position will work in an integrated team that includes, Behavioral Health therapists, Registered Dietitians, Nurses, Primary Care Medical Providers and Medical Assistants, and will work collaboratively with community Obstetrical Providers.

    The position affords the opportunity to build a healthier community by providing outstanding, culturally sensitive, comprehensive women's health care services.

    ROLE AND RESPONSIBILITIES

    • Provide high-quality women's health care, including well women visits, according to the standards of the ACOG; with particular attention to routine screening tests, health maintenance, anticipatory guidance and collaborative care for patients with psycho-social problems

    • Ensure timely referral to community OB/GYN for continued prenatal and obstetrical care, after completing the initial parental visit

    • Collaborate with the community OB/GYN group on any shared medical care

    • Work closely with other team members within the department, site, and organization to accomplish assigned goals, objectives, and responsibilities

    • Ensure compliance with CHC standards including those established by the Joint Commission, State of Connecticut licensure, and CHC internal quality committees, OSHA/Environment of Care, Safety/Infection Control, Quality Care Audits, and DPH licensure requirements

    • Adhere to CHC standards for Electronic Health Record charting and documentation

    • Work in a team atmosphere with other members of the clinical team including participation in daily huddles, periodic team meetings and sharing responsibility for improving outcomes with a medical team consisting of primary care provider, nurses, and medical assistants

    • Works collaboratively with the Healthy Start Outreach Worker and Healthy Start Care Coordinator to ensure smooth transition to ongoing prenatal care with a community OB/GYN group and continue regular communication and collaboration

    • Demonstrate commitment to professional development, maintenance and enhancement of skills through participation in continuing professional education activities

    • Actively participate in Quality Initiatives identified that effect the Women's Health department

    • Represent CHC at specific trainings, meetings and/or conferences, which may require travel, as required by the Healthy Start Grant. This may include Department of Public Health (DPH) and community meetings

    • Provide DPH with defined operational and clinical data as required by the Healthy Start Grant

    QUALIFICATIONS

    Required Licenses/Certifications

    • Licensed or license eligible in State of Connecticut

    • Current DEA license

    • Current CPR certification

    Required Skills and Education

    • Board Certified as a Women's Health Nurse Practitioner or Certified Nurse Midwife

    • Have a minimum of three (3) years of experience as a Women's Health Nurse Practitioner or Certified Nurse Midwife

    • Commitment to care of under served populations

    • Excellent interpersonal, written and oral communication skills

    • Computer skills required

    • Bilingual (English/Spanish)
