    Description:

    Emergency Room Function

    Identification of patients who are registered to be seen in the Ellis Medicine Emergency Room and who are either not connected to a primary care provider or established with an Ellis Medical Group Primary Care Physician. Once these patients are identified the care manager would:

    • Identify the chief complaint/reason for emergency room visit

    • Identify the patients PCP and review records for chronic disease management opportunities

    • Review disease specific zone sheets for COPD/CHF/Diabetes, etc.

    • Conduct a psycho-social assessment with the patient and or healthcare representative

    • Screen the patient for SIOH- Mitigate using appropriate resources

    • Review for ambulatory care sensitive conditions

    • Review the "levels of care" zone sheet if appropriate and the patient presented with an ACSC

    • If the patient is discharged from the Emergency Room, make a follow up appointment with the patients PCP

    • If the patient has COPD/CHF/Diabetes, follow patient for a 30-day period of time to complete a 48 hour follow up call, med rec, ensure the patient filled any prescriptions written, etc.

    Education Requirements :

    RN Required, BSN preferred (NYS Registered Professional Nurse licensure) or equivalent education and experience.

    Experience Requirements :

    5 years of RN experience in Care Management preferred. Prior electronic medical record (EMR) experience required. Previous project leadership experience preferred.

    Pay Range:$31.50 - $44.95

    Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
