Highlights

Community Health Network is seeking a Psychiatric-Mental Health Nurse Practitioner (PMHNP) to join the Community Behavioral Health team in the Community Mental Health Center setting. This provider will practice within two of Community's outpatient clinics located in Kokomo and Tipton, Indiana, and will work with the adult population. This position also entails providing medication-assisted treatment (MAT). Ideal candidates will be highly accountable, independent, and have a desire to deliver direct, collaborative patient care.

Licensure & Certification

Educational background consistent with requirements for licensure and certification within the state of Indiana.
