Community Physician Network is currently seeking a Family Medicine physician to practice in our outpatient clinic near Community Hospital North on 96 th Street in Indianapolis, IN.

• Outpatient, well established practice

• No OB

• Shared call schedule with 24/7 RN triage

• Electronic Medical Record (Epic)

• Large group of specialists based at Community Hospital North

Licensure & Certifications

• Board Certified or Board Eligible in Family Medicine

• Licensed in Indiana or eligible for medical licensure in Indiana
