Physical Therapist / PT - PRN

GENERAL PURPOSE

Performs evaluations and develops effective patient treatment plans to restore, maintain or prevent decline of patient function, by planning and administering medically prescribed therapy treatments in accordance with federal, state and professional standards governing the treatment location.

• Hours: PRN / Flexible

• Setting: Home Health with options for Skilled Nursing Facility & Outpatient

• Location: Good Shepherd Lutheran Home Health - Sauk Rapids, MN

QUALIFICATIONS

• Graduate from Physical Therapy program

• Current license as a Physical Therapist or ability to obtain in the state of practice

• Must meet Clinical Competency requirements

• Current CPR certification, preferred
