    Description:

    POSITION PURPOSE

    PGY2 pharmacy residency programs build on Doctor of Pharmacy (PharmD) education and PGY1 pharmacy residency programs to contribute to the development of clinical pharmacists in specialized areas of practice. PGY2 residencies provide residents with opportunities to function independently as practitioners by conceptualizing and integrating accumulated experience and knowledge and incorporating both into the provision of patient care or other advanced practice settings. Residents who successfully complete an accredited PGY2 pharmacy residency are prepared for advanced patient care, academic, or other specialized positions, along with board certification, if available.

    The purpose of the PGY2 pharmacy residency in critical care is to provide an experience which leads to the development of expert knowledge and skills in critical care pharmacy practice. The PGY2 pharmacy residency will prepare the resident for job opportunities in the following areas:

    1. Critical Care Pharmacist in hospital setting

    2. Clinical Pharmacy Specialist

    3. Non-Tenure Track College of Pharmacy Faculty

    REQUIRED EDUCATION, EXPERIENCE AND CERTIFICATION/LICENSURE

    Education: Graduation from an ACPE-accredited College of Pharmacy with a Doctor of Pharmacy Degree

    Experience: Completion of an ASHP-accredited PGY1 Pharmacy Residency

    REQUIRED SKILLS AND ABILITIES

    1. Valid Controlled Substances License or eligible for a controlled substance license in the State of Michigan within 30 days from date of hire.

    2. Valid Pharmacist License or eligible for pharmacist licensure in the State of Michigan within 30 days from date of hire.

    3. Current Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) certification

    4. Occasional professional travel will be required

    5. May have occasional contact with caustic or volatile chemicals and medications, or other hazardous materials with special precautions

    6. Must follow basic infection control practices while working in a patient care environment which may result in exposure to communicable diseases.

    7. Must be able to work effectively in an environment of high pressure, stress, noise and confusion while ensuring accuracy in interpreting, evaluation, preparing, calculating, and dispensing appropriate medication pursuant to a medication order.

    ESSENTIAL FUNCTIONS AND RESPONSIBILITIES

    1. Completes the competency areas, educational goals, and objectives as set forth by the American Society of Health-System Pharmacists (ASHP) accreditation standards and established for the PGY2 Critical Care Pharmacy Residency Program for each rotation.

    2. Provides direct pharmaceutical care to the patient populations served; identifies, prevents and resolves drug related problems to promote safe and effective use of medications; monitors patient progress to support positive outcomes; and documents (as required) medication interventions.

    3. Completes one major longitudinal research project during the residency year and other minor longitudinal projects while on rotation.

    4. Obtains and/or maintains additional advanced certifications (e.g. ACLS, PALS, Teaching Certificate) if not already obtained during PGY1 Pharmacy Residency

    5. Participates in Code Blue, Rapid Response Team, Code Sepsis, and/or Stroke Team pages as available to provide medication management suggestions during emergency patient care situations

    6. Completes and presents at least one accredited continuing education (CE) presentation

    7. Demonstrates and understanding of critical care-specific disease states and conditions, in order to apply evidence-based medication recommendations to direct patient care at the bedside

    8. Provides a safe and effective medication use process; procures, compounds, prepares and dispenses medications according to departmental policies and procedures.

    9. Provides functional supervision to pharmacy operations in assigned work area. This includes responsibility and accountability for ensuring timely, accurate completion of pharmacy workload during assigned shift. Directly supervises pharmacy technicians while in operational staff roles.

    10. Participates in continuous quality improvement activities/teams to meet departmental or institutional goals. Prepares reports, educates, and makes suggestions and recommendations for improvement. Develops protocols, policies and practices.

    11. Performs other responsibilities and duties as assigned by the residency director or coordinator, or the individual rotational preceptors.

    12. Provides drug information services to patients and professional staff to assure appropriate use of medications.

    13. Participates in evaluating medications for formulary consideration, with a pharmacoeconomic analysis and prescribing guidelines, in conjunction with the appropriate preceptor and the P&T Committee.

    14. Participates in relevant hospital and pharmacy committees as part of the rotational or longitudinal research project activities; establishes collaborative practice relationships with other healthcare providers.

    15. Participates in local, regional and/or state professional organizations such as SCCM, ASHP, SMSHP, MSHP and MPA.

    16. Participates in the education of undergraduate pharmacy students and provides educational sessions to pharmacy, nursing and medical staff as appropriate and while on rotation.

    17. Demonstrates a commitment to the Department of Pharmacy vision and mission and principles of continuous quality improvement within the organization.

    18. Maintains good rapport and cooperative relationships. Approaches conflict in a constructive manner. Helps to identify problems, offer solutions, and participate in their resolution.

    19. Maintains the confidentiality of information acquired pertaining to patient, physicians, associates, and visitors to St. Joseph Mercy Health System. Discusses patient and hospital information only among appropriate personnel in appropriately private places.

    20. Behaves in accordance with the Mission, Vision and Values of SJMHS.

    21. Assumes responsibility for performance of job duties in the safest possible manner, to assure personal safety and that of coworkers, and to report all preventable hazards and unsafe practices immediately to management.
