Patient Care Advocate - Remote

Description

• Employee will be performing member outreach.

• Coaching members into healthy habits and gap closures. regarding coverage and provider visits.

• Assist with membership attribution

• Assist with provider and patient engagement

• Assist with solving for SDOH concerns

• Provide resources to members to improve health outcome

Qualification

• LPN or Medical Assistant ( Nurses must have an active compact license for TN,MS, or AR)

• Clinical experience working in a physician's office

• Previous clinical outbound call outreach; motivational interviewing

• Case Management/Utilization Management

• HEDIS
