Management of Facilities Based Care Management

Job Description

Insight Global is looking for a strong Manager of Care Management to join their team. This person must have strong communication, the ability to develop/train teams, and be available to any questions from the field. Each day will start with a huddle to level set the day, and comprised of approving discharge planning tools, DME, planning transportation, answering phone calls, denials, issuing Medicare notice, and available and ready to help the team whenever it is needed. This person will be expected to go onsite five days a week. Previous experience as a manager in an acute care/inpatient hospital setting with diversity of experience in subacute, out of network and inpatient programs is a must. This person will be in charge of managing an 8 person team within inpatient, sniff, and out of area.

Skills and Requirements

• 3+ years of managerial experience (mid-sized team) in an acute care/inpatient hospital setting

• Subject matter expert on the acute side of case management; experience with health plans: Medicare, Anthem, Blue Shield/Blue Cross, Cigna, etc.

• Strong hands-on management experience with expertise in developing/training; diversity within subacute, out of network, and inpatient programs

• Familiarity with MCG Care Guidelines, MS Suite, DHC tools, and utilization management practices

• RN License - Cerner experience

• Case Management certification null
