    Case Managers help patients understand their options concerning the specific situation they are

    dealing with at the time. They are liaisons between patients and their treatment or care options.

    Shift Details: Full time, 40 hours - 8:00-4:30 office hours. Possible on call duties

    Additional Shift Details: Case management duties. Case conferencing with field caregivers. Collaborating/calling doctor's clinics, hospitals, rehabs, etc. Assist field caregivers in meeting patient needs/goals.

    Location: Southern Nevada (Las Vegas, Henderson, Boulder City, Mesquite, Laughlin)

    QUALIFICATIONS:

    1. Graduate of an accredited school of nursing. Previous home health/OASIS experience is a plus

    2. Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist

    3. Have a sympathetic attitude toward the care of the sick and elderly

    4. Ability to carry out directions, read and write

    5. Current licensure in state, CPR certification and valid drivers license

    6. Current physical and TB test required

    RESPONSIBILITIES:

    1. Coordinate and provide care that is safe, timely, effective, efficient, equitable, and client centered

    2. Handle case assignments, draft service plans, review case progress and determine case closure

    3. Help clients achieve wellness and autonomy

    4. Facilitate multiple care aspects (case coordination, information sharing, etc.)

    5. Help patients make informed decisions by acting as their advocate regarding their clinical status and treatment options

    6. Develop effective working relations and cooperate with medical team throughout the entire case management process

    7. Take the extra mile and interact with patients to keep track of their progress and to ensure satisfaction

    8. Record cases information, complete accurately all necessary forms and produce statistical reports

    9. Oversee office staff within the IMH Department (job performance and time management as well as PTO requests)

    10. Promote quality and cost-effective interventions and outcomes

    11. Assess and address motivational and psychosocial issues

    12. Adhere to professional standards as outlined by protocols, rules and regulations

    13. Attends in service and continuing education programs as scheduled and necessary

    14. Attends patient care conferences as scheduled

    $27.60 - $42.57
