    ONCALL Hospice Visit RN

    The Community Hospice has a wonderful opportunity for a Per Diem RN to work as part of our Hospice Care Team caring for patients and families serving Capitol Region/Saratoga/Washington/Montgomery/Columbia and Greene Counties.

    Position Highlights:

    What you will do:

    • You will visit patients in the home setting and local nursing homes to respond to symptom management needs and coordinate with our team to provide added support to patients and families.

    Responsibilities:

    • Based upon the assessment of the On Call Triage RN, this On Call Visit RN will be called to care for a patient/family in need

    • Service as an advocate in the ER as needed for urgent needs/pronouncement.

    • Work with interdisciplinary team.

    • Hours needed during the week are afterhours- evenings and overnights. Hours needed on the weekend are mainly days.

    What you will need:

    Minimum Requirements

    • Valid NYS RN license required.

    • 1-2 years previous acute care RN experience, 1-2 years homecare and/or Hospice experience preferred.

    • Ability to learn/use an electronic medical record

    • Clean Driver's License and reliable vehicle

    • Strong interpersonal skills to establish and maintain relationships with patients, families, physicians and coworkers
