The Home Health Aide is a member of the home care team who works under the supervision of a registered nurse and performs various personal care services as necessary to meet the patient's needs. The home health aide is responsible for observations and documenting observations and care performed. The home health aide will be assigned in a manner that promotes quality, continuity and safety of a patient's care.
Primary Responsibilities:
Responsibilities of the home health aide are outlined in the plan of care, including but not limited to:
Baths
Oral Hygiene
Shampoos
Changing bed linen
Assisting patients with dressing and undressing
Skin care to prevent breakdown
Assisting the patient with toileting activities
Taking and recording oral, rectal and axillary temperatures, pulse respiration and blood pressure when ordered
Assisting in ambulation and exercise according to the plan of care
Performing range of motion and other simple procedures as an extensional therapy service as ordered
Remind patient in the self-administration of medication
Meeting safety needs of patients and using equipment safely and properly (foot stools, side rails, etc.)
Reporting on patient's condition and significant changes to the assigned nurse
Adhering to the Agency's documentation and care procedures and standards of personal and professional conduct
Communication

Maintains a patient case load, daily visits and point of care documentation levels as per Agency standards
Demonstrates personal responsibility with regard to attendance and punctuality
Maintains privacy and confidentiality with regard to all patient, staff and agency information
Demonstrates flexibility, enthusiasm and willingness to cooperate while working with others or in place of others, as necessary
Expresses verbal and written communication in a clear, positive and cooperative manner
Promotes the agency image by adhering to the Dress Code
Performs all other duties as assigned

Required Qualifications:
High School diploma or equivalent
Ongoing attendance at education/in-service programs as deemed necessary by management
Current Certified Nursing Assistant certification issued by the Texas Health and Human Services and a person who has successfully completed a State-established or other training program that meets the Texas requirements
Valid Texas Driver's License and maintain personal auto insurance coverage
Current CPR certification
1+ years of experience in health care
Experience working with electronic medical record application
Proficient in English with written and oral communication
Thorough knowledge of current Nursing Aide practice, professional standards of care, and state and federal regulations regarding home care
Demonstrates excellent observation, verbal and written communication skills, problem solving, basic math skills, and nursing skills per competency checklist
Proven ability to comply with accepted professional standards and practices
Proven basic PC skills to include Window, Outlook and Other email systems
Proven ability to prioritize and communicate objectives clearly
Must not register as unemployable on the Texas Nurse Aide Registry nor the Texas Employee Misconduct Registry
